I need a good fitness regimen?

I'm a 30 something year old female, no kids but after years of being a gym rat, it fell off due to medical conditions. I am looking at toning up and loosing up to 10lbs but I mostly concerned about toning. Due to my medical conditions, I'm wondering is there a video I could do at home or exercises to do at the gym that are not to weight baring and cause more pain than I am already in daily but get results?
